על לידות בית

נושא שנוי במחלוקת אשר מובא לדיון באמצעות דבריו של פרופ' עידו שולט: "יש לשקול היטב את הסיכונים אל מול הסיכויים טרם קבלת החלטה"

נושא לידות הבית חזר לדיון ציבורי בישראל גם בעקבות מסמך NICE מדצמבר 2014 באנגליה בו הומלץ ליולדות בסיכון נמוך לשקול לידות בית היות ובקבוצה זו הודגם שיעור פחות של לידות מכשירניות וניתוחים קיסריים....
נושא זה שנוי במחלוקת, לרבים מהעוסקים בו דיעה ברורה לכיוון זה או אחר וכפועל יוצא מכך הדיון בו בעייתי. עיקר הדיון מתמקד בחופש הבחירה, העומד על כפות המאזניים אל מול הסיכונים.
בהולנד, בה שיעור לידות הבית הוא הגבוה בעולם המערבי, ירד שיעור לידות הבית מ-35% בשנים 1997-2000 ל-23% מ-2009.
במאמר מטהאנליזה, הגדול ביותר שפורסם עד כה, שפרסם WAX ב 2010 בעיתון האמריקאי AJOG, הושוו תוצאות לידות בית ללידות בי"ח. במחקר נדונו תוצאות מחקרים מארה"ב, קנדה, הולנד, שבדיה, אנגליה שוויץ ואוסטרליה. נסקרו 12 מחקרים, מעל 342,000 לידות בית מתוכננות הושוו למעל 200,000 לידות בי"ח מתוכננות. לידות בית נמצאו קשורות לפחות התערבויות כגון ניטור במהלך הלידה, שיכוך כאב אפידורלי, חיתוך החיץ ולידה מכשירנית אך זאת במחיר של כמעט פי שלוש תמותת ילודים. שיעור מקרי התמותה הנאונטלית (לאחר הוצאת הילודים שנולדו עם מומים קשים מהמחקר) היה גבוה פי 2.87 עם רווח בר סמך של 1.32 עד 6.25.
משרד הבריאות מאפשר לידות בית ליולדות בסיכון נמוך. במאמרו של עמוס גרינבאום שפורסם במרץ 2015 באותו העיתון, הודגם כי למרות המגבלות, בכ-30% מלידות הבית בארה"ב נמצאו גורמי סיכון משמעותיים כגון מצג עכוז, מצב לאחר ניתוח קיסרי או לידות תאומים.
אין חולק על חופש הבחירה ליולדות אך יש לשקול היטב את הסיכונים אל מול הסיכויים טרם קבלת החלטה.

נתון חשוב שיש לקחת בחשבון בלידות בית בארה"ב הוא שלידות בית רבות מתרחשות בקרב נשים ממעמד סוציואקונומי נמוך, כיוון שלידה בבי"ח לאשה ללא ביטוח רפואי, יקרה בעשרות מונים מלידה בית. ברבים מהמקרים מדובר בנשים עם מעקב הריון דל וילודים שבמקרים רבים נמצאים בסיכון מעת לידתם.
